Pre-Screening Questions

Prior to scheduling an appointment to donate blood, you will be asked the following pre-screening questions. These questions help to ensure the safety of the community blood supply and make the donation process to be more efficient. Click here for full details on donor eligibility.
Do you weigh less than 110 pounds or more than 350 pounds?
Are you 15 years or younger?
Are you ill or not feeling well today?
Are you taking antibiotics today for an active infection?
Have you taken Accutane, Proscar or Propecia in the last month?
Have you taken Coumadin/warfarin in the last 7 days?
Have you received a piercing or tattoo in the past 12 months?
Have you ever been diagnosed with lymphoma and/or leukemia?
Are you currently receiving treatment for a diagnosis of cancer?
If yes, is the treatment for cancer other than - basal cell,
squamos cell skin cancer or carcinoma in situ (any size)?
Have you ever had leishmaniasis?
In the past 12 months, have you traveled to Afghanistan, Africa, Costa Rica, Dominican Republic, Haiti, India, Iraq, Malaysia, Mexico or Vietnam?
If yes, while in Afghanistan, Africa, Costa Rica, Dominican Republic, Haiti, India, Iraq, Malaysia, Mexico or Vietnam did you travel to any of the countries or cities listed in Table 1 below?

If you answered “yes” to any of the questions above you may be either temporarily or permanently ineligible to donate blood. Click here for additional information about donor eligibility.
These questions are for pre-screening only. Even if you answered "no" to all of these questions at the time of pre-screening, you may be ineligible to donate based on the answers given during the questionnaire and interview process completed just prior to the actual donation.
